Healthcare mail already has consumers' attention: Only 20% of consumers say they typically throw healthcare mail away, and nearly half read it as soon as it arrives. But attention alone isn't enough. Eighty-three percent of consumers say they're more likely to engage when mail feels relevant – whether it's timely, personalized, or connected to a current healthcare need. This report explores what today's healthcare consumers want from direct mail and how brands can deliver it.
Study Methodology
Lob conducted this study in partnership with Kirin Analytics and PureSpectrum. The online survey was completed in July 2026 with 2,000 adult consumers across the United States, nationally representative of age, gender, income, and region.
Economic pressure is changing what consumers expect from healthcare mail
Sixty-eight percent of consumers told us economic conditions have changed what they pay attention to in their mailbox this year. When budgets are tight, consumers look for communications that can help them save time, understand their benefits, or make informed healthcare decisions.
Economically strained consumers particularly want appointment reminders, useful branded items, and promotions or perks.
Direct mail also builds trust among this group. 42% of consumers who feel financially strained say receiving direct mail from a healthcare brand makes the brand feel more credible, compared to just 16% of less-impacted consumers.

Healthcare mail creates the most value when it helps patients navigate their care
Consumers are most likely to engage with mail that reflects a current need, arrives at the right time, or provides information that's specific to their care. In fact, 83% say at least one relevance-related factor makes them more likely to read healthcare direct mail.

When it comes to healthcare mail, consumers prefer formats that feel familiar and easy to engage with. Traditional letters remain the clear favorite, growing from 34% to 40% year over year, while postcards also gained traction. The takeaway is simple: healthcare marketers don't need elaborate packaging to capture attention. A straightforward format that delivers timely, relevant information is often the most effective.
